We would be grateful if you would fill in this form. It does not form part of your application and is not used for any purpose other than to monitor and gather diversity information of applicants through our different scholarship programmes recruitment. We do not ask for your name on the form.

This information is being used to inform our own monitoring and strategy and will not be used as the basis of individual funding decisions.

Do you look after or care for someone with long term physical or mental ill health caused by disability or age (not in a paid capacity?)

If you are accepted onto this programme and you think you need a reasonable adjustment for example because you are a carer or have a disability or if you want to share anything else please talk to Fiona Moss.
